I worked a made up patient, a 19 year old with fever, a cough and pain on breathing in. I wrote my top three guesses in order with a reason for each, named the test I wanted, and said what I would do right now, all before finding out the answer.

Patient: a 19 year old with three days of fever, a bad cough and pain in her right side when she breathes in. My guesses, in order: 1. Pneumonia in the right lower lobe. Fever, a wet cough and pain only on breathing in all point at the lining of the lung. 2. A clot on the lung. The same pain fits it, and missing it is the one that kills her. 3. Pleurisy after a virus. The one test I want: a chest x-ray. What I would do right now: check her oxygen and start antibiotics.
